Initial Evaluation

For one time evaluation
Covers your first session and assessment. Follow-ups are charged separately.
Follow-Up Sessions
$150-$185
Per follow - up session
Ongoing support to help you stay on track and continue your progress.
Coaching
$300
For one time evaluation
Road Map to Success: A one-time evaluation session designed to help you clarify goals, identify obstacles, and outline a personalized action plan.
Cancellation Fee
$75
No - show or cancellation
This fee applies for no-shows or cancellations made less than 48 hours prior to the appointment.
Good Faith Estimate
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

You Have The Right
You have the right to receive a Good Faith Estimate for the total expected cost of non-emergency services, including medical tests, prescription drugs, equipment, and hospital fees.

Get an Estimate
Request a written Good Faith Estimate from our health care team at least 1 business day before your medical service or item. You can also ask our team for a Good Faith Estimate before scheduling.
03.
You Can Dispute
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
04.
Save a Copy
Save a copy or picture of your Good Faith Estimate. For questions or more information, visit www.cms.gov/nosurprises or call 800-985-3059.
