- Affected individuals will be notified in writing without unreasonable delay, and no later than sixty (60) days from the date the breach is discovered.

- If the breach affects 500 or more individuals, we will notify the U.S. Department of Health and Human Services (HHS) and prominent media outlets in the affected area as required by law.

- If the breach affects fewer than 500 individuals, we will report it to HHS as part of our annual breach log submission.

- We will also comply with the New Jersey Identity Theft Prevention Act notification requirements where applicable.
